Unfortunately, we are not able to accommodate walk-in appointments, we ask that you call or email to make an appointment.

You will receive a text or email from our office once your appointment has been scheduled. Please follow the link to complete the registration forms. Please bring your insurance card and photo ID. Bring any pertinent medical records and medical imaging.

Our Physicians accept all major PPO insurances with the exception of HealthNet and Aetna. Dr. Gregorius does not accept United Healthcare. We do not take any HMO insurances.  Dr. Mikulak is not contracted with Medicare.

No, the office does not check benefits to verify coverage prior to your visit. It is advised that you verify your health care benefits to avoid any unexpected charges.

We perform X-ray imaging on-site. We refer out for any additional imaging such as MRI’s, CT’s, etc…

Please give all medication refills a 48-hour turnaround. Medications will be refilled during normal business hours. Opioids will not be dispensed on the weekends or holidays.
